Below is a list of the incident types that would be reported as part of a PSMS and an overview of PSMS and CAP.

Note: this list is not exhaustive. For more information, please contact the Quality Department.

Damages

  • At-fault gas line hits
  • Not at fault gas line hits
  • Cross bore or sewer damage discovered during construction
  • Cross bore or sewer damage discovered after construction
  • Un-safe excavation practices by Miller crews
  • Un-safe excavation practices by others

 

Existing gas lines that will remain in service

  • Gas lines discovered with insufficient cover (extremely shallow or in areas where excavation is likely)
  • Gas lines that are exposed (due to erosion)
  • Gas lines that can’t be located
  • Gas lines with visible corrosion, diminished wall thickness or sub-standard cathodic protect levels.
  • Meter sets or above ground gas lines exposed to vehicular damage
  • Fittings, connections or pipe types that have been outlawed
  • Underground leaks (if discovered doing work other than leak repair)
  • Above ground gas leaks (if discovered doing work other than leak repair)

 

Miller Pipeline-installed gas lines

  • Lines not installed to customer specs, caught and corrected by us during installation
  • Lines not installed to customer specs, caught by others after installation
  • Leak test failure – found and corrected by Miller at time of installation
  • Leak discovered on Miller install – found by others, after installation complete
  • Fusion not acceptable – found and corrected by Miller during installation
  • Fusion not acceptable – found by others after installation
  • Covered task performed by Miller employee without proper OQ’s – caught and corrected during installation
  • Covered task performed by Miller employee without proper OQ’s – discovered after the fact, by others
  • Defective materials – found and corrected by Miller during installation
  • Defective materials – found by others after installation

 

Non-compliance issues

  • Tools and devices used outside parameters of calibration or inspection – caught and corrected during installation
  • Tools and devices used outside parameters of calibration or inspection – discovered after installation
  • Work performed without valid permit/license – fusion card, welding permit, plumbing permit, etc.
