Utility Damage

The Utility Damage Report is to be completed when there has been damage to a gas line, electric line, water line, cable television line, telephone line, etc.
Incident Report

The Incident Report is to be completed when Miller Pipeline equipment/vehicles have been stolen or damaged or there are accidents involving two Miller Pipeline vehicles/equipment without injuries (ex. a Miller Pipeline backhoe striking a Miller Pipeline vehicle).
Public Injury

The Public Injury Report is to be completed when there has been an injury to a non-Miller Pipeline employee.
First Report of Injury (Injury Hotline)
The First Report of Injury form is to be completed when there is a work-related injury to a Miller Pipeline employee. However, it should be noted that the Injury Hotline will complete the form on the injured employee’s behalf. Please call the Injury Hotline at (888) 624-4289.
Vehicle Accident
The Vehicle Accident form is to be completed when there is a motor vehicle accident involving a Miller Pipeline vehicle and a non-Miller Pipeline vehicle.
However, it should be noted that Fleet Response will complete the form on behalf of the Miller Pipeline driver. Please call Fleet Response at (800) 338-0619. Click here for the procedure guide.
