First Report of Injury

If a life threatening injury occurs, call 911 and notify your superintendent immediately.
If a non-life threatening injury occurs (even if it is a minor injury), call our "Injury Hotline" at 888-624-4289 and notify your superintendent immediately.
When you call our "Injury Hotline" all necessary paperwork will be completed for you.
If you do not call our "Injury Hotline" for some reason, you will need to complete the "First Report of Injury" form. The link below will take you to this form.
